| Primary Function: & Major Responsibilities |
PRIMARY FUNCTION: - Provides all necessary primary care services to patients at the Hospital who have been assigned to the Behavioral Health area of the Hospitalist Service. Provides consultations for requesting physicians in other areas of the hospital on a timely basis. MAJOR RESPONSIBILITIES: 1. Provides primary care and consultative services to patients assigned to the Hospital Medicine Service. Standards: a. Principle responsibilities include acting as Primary Care provider managing the medical needs of patients in the Chemical Dependency unit. b. Provides medical consultations for patients on the Psychiatric unit at the request of the Psychiatrists. 2. Ensures referred and assigned patients are properly and efficiently admitted and receive appropriate and responsive medical care according to their condition, symptoms, and diagnosis. Standards: a. Develops plan of care in conjunction interdisciplinary staff to ensure quality care of patients. b. Provides patient and family members with proper information regarding care plan and conducts care conferences as appropriate. c. Works closely with counselors and nurses to ensure care is delivered timely and appropriately according to each patient’s individual need. 3. Completes effective and timely verbal communication with the patient’s primary physician at various stages during the process, such as admission, care conferences with interdisciplinary team & families and upon discharge. Standards: a. Assists in obtaining pertinent information from primary care and referring physicians. b. Proactively communicates with other clinicians in order to affect a coordinated and timely transfer of information of necessary information to improve the quality of care for patients. c. Documents all significant communications in the patient’s chart or through other appropriate mechanisms. d. Dictates discharge summaries to primary care physicians within 12-24 hours of discharge to allow for timely communication after discharge. 4. Functions as a member of the Clinical Care Team to achieve patient outcomes within established parameters, through the development, communication and the implementation of the plan of care. Standards: a. Utilizes all interdisciplinary care team members and their resources or expertise to develop and implement the plan of care. b. Returns pages and phone calls in a timely fashion to clarify orders, arrange discharges, follow-up on tests, and answer patient and family questions. c. Works closely with the Care Coordinator to ensure the details needed to achieve these outcomes is in place. 5. Documents patient diagnosis, treatment, orders and clinical progress properly and correctly in order to allow for proper and correct billing. Standards: a. Attends all required educational sessions related to patient documentation and reimbursement and demonstrates thorough knowledge of this area when documenting individual patient information. b. Uses approved abbreviations and meets all required medical staff rules and regulations regarding documentation in the clinical record. c. Documents H&P information to allow for complete collection of co-morbidity and complication information for use by hospital medical records and coding staff. d. Times and dates all progress notes. Prints signature every time. e. Meets with Coding staff regularly and at request to review coding issues. Works closely with them to ensure appropriate billing for each encounter. f. Submits charges on a daily basis. 6. Functions as a spokesperson for Hospitalist program and for HealthEast in a variety of settings, both internal and external, to improve awareness of service line and benefits to patient, physicians, the community and the hospital. Standards: a. Demonstrates confidence and competency in the role of Behavioral Health Hospitalist to patients, staff members and others to enhance understanding of program and the benefits. b. Describes aspects of the Hospitalist program to clinical staff and referring physicians as needed. 7. Provides care to patients in other settings as required to ensure quality and the continuum of patient care while ensuring patient safety and satisfaction. Standards: a. Sees patients in the outpatient setting for follow up after the completion of chemical dependency treatment. b. Responds to request for chemical dependency evaluation needs within the hospital as requested. c. Serves on related hospital committees to bring perspective of service into policies and procedures with emphasis on quality improvement, clinical advantage and operational effectiveness and efficiency. 8. Participates in Graduate Medical Education programs including serving as teaching service faculty as requested by the Hospital Medical Director. Standards: a. Identifies and assigns patients to rotating resident physicians designed to provide educational opportunities appropriate for level of training. b. Meets on a regular basis with rotating residents and other assigned students to review and supervise patient care and meet all requirements in the educational agreements and Rules and Regulations. c. Provides timely feedback and evaluations to faculty and medical staff leaders regarding resident and student progress and potential and actual quality of care concerns. 9. Works closely with the Operations Manager of Hospital Medicine or designee to ensure the Hospitalist Service meets and exceeds the needs of its customers. Standards: a. Frequently communicates with the director regarding census, assignment of patients to the service and any customer service needs. b. Cooperates in the development of a work/call schedule that secures 365 days/year coverage. c. Collaborates with the Operations Manager to assist in the continual improvement of the service and actively participates in strategic planning for growth and development. 10. | Qualifications: | QUALIFICATIONS: EDUCATION: - Valid MD License in the State of Minnesota. Board Certified or Board Eligible in Family Medicine by the American Board of Family Medicine or Internal Medicine by the American Board of Internal Medicine. Addiction Medicine trained preferred. Active DEA registration certificate including buprenorphine waiver. EXPERIENCE: - Demonstrated competence in practicing medicine. Hospital based inpatient management experience preferred. Experience working with patients who have chemical dependency and/or mental health diagnosis preferred. Previous experience working in a total quality improvement environment preferred SPECIAL KNOWLEDGE, SKILLS, AND ABILITIES: 1. Demonstrated clinical expertise. 2. Demonstrated organizational, problem solving, time management, and conflict resolution skills. 3. Current clinical skills regarding management of medical care and use of equipment. 4. BLS and ACLS. 5. Excellent interpersonal skills to work successfully with all disciplines including medical and support staff. 6. Clear understanding of Care Management and Performance Improvement approaches to clinical practice. 7. Knowledge of health care management and the hospital care environment. 8. Demonstrated leadership skills. 9. Knowledge of local healthcare market. 10. Previous experience with developing and maintaining effective medical staff relationships. 11. Effective liaison with HealthEast Medical Staff. 12. Extensive PC knowledge and skills. 13. Excellent verbal and written communication skills. 14. Demonstrated ability to work independently. 15.
